• Date, Time, location and nature of the event (eg. fund raiser)
• Contact person name and phone number or email address
• What role would they play (eg. speaking, presenting)

We will contact you to confirm attendance or extend regrets.

Please submit your request in writing, if possible.

Be sure to include:

• Your name and contact information
• Full name(s) and address for the recipient(s)
• Address you’d like the certificate sent to (let us know if you’d like to pick it up)
• Details of the occasion, including the specific date of birth or wedding as well as the date on which any celebration will occur
• Date when certificate is needed

Please submit your request in writing, if possible.

Be sure to include:

• Your name along with that of the organization or group you represent
• Let us know if it is a non-profit organization, or for a City-sponsored event
• Your contact information, address and phone number
• Purpose of the event or circumstances, ie. the reason for the request
• How many pins you require
• When you would like to pick them up

Please note that lapel pins cost the City 36¢ each, taxes included. Due to costs we reserve the right to limit the quantities given away.

Please submit your request in writing, if possible.

• The name of the organization and related information about it
• Detailed background information about the event, including dates & locations
• How you would like to receive the message, (eg. by email)
• Your printing deadline
